Notice of Data Breach

On December 2, 2020, we detected suspicious activity within our business email environment. A comprehensive investigation was conducted by a third party forensics firm to identify whether any sensitive data had been compromised. Through this investigation, it was determined a certain business email account had been compromised by an unauthorized third party. Upon detection, the email environment and all accounts were secured.

We found no evidence that specific individuals’ information has been misused, however, it is possible that the unauthorized third party could have viewed an individuals’ full name, mailing address, and one or more of the following: social security number, date of birth, driver license or state identification information, passport, medical information, health insurance information, and financial account information.

Due to this compromise, we mailed notification letters to all individuals whose information was available for compromise during the incident. The notification letters contain additional information about what occurred, along with a toll-free number to our call center, which individuals can call to learn more about the incident and how to protect their information.

We are also offering free credit monitoring and identity theft protection services for twelve months to potentially affected individuals through a third party vendor, Kroll. Potentially affected individuals may contact our call center, managed by Kroll, at (855) 545-0730. Kroll is available Monday through Friday between 6:00 AM and 3:30 PM ET.

As of this writing, we have not received any reports of related identity theft since the date of the incident (December 2, 2020).
